Compassionate Neighbours Project
Running for 7 years, this is an integral part of St Joseph's Hospice. It was set up as a social movement to create compassionate communities and has now expanded to 13 hospices in and around London. We provide local people with training who then provide friendship to those struggling with loneliness and isolation due to their conditions or are at the end of life.

The Prince's Trust (support for 16-30 year olds)
We provide education, training and employment support for 16-30 year olds. We help people build confidence, get jobs and qualifications and to start their own businesses, and much more. We have a local centre in tower hamlets and work across all the main industries and with a host of partners.

Adult Social Care Strategy and Vision
When people think of social care they often think of things like home care, care homes and day centres. But adult social care is much more than that. Support can range from advice services designed to help people solve issues at an early stage, to employment support for people with a disability.

Information, Advice & Early Help
Tower Hamlets Connect is an integrated service, funded by Council and NHS partners to provide advice information, early help. The Tower Hamlets Connect portal provides online advice and info, signposting, events and a community directory. We will also use the stall to talk about the joint work we are doing around frontline integration of advice and info.

Continence Service
The service is provided by specialist continence nurses (SCN) with advanced knowledge of bladder and bowel dysfunction. We offer advice, training and an advanced specialist clinical service in the prevention and treatment of incontinence. The aim of the continence service is to promote continence, improve or achieve continence and to manage incontinence for any adult experiencing bladder/ bowel dysfunction with the community. We manage patients with complex bowel and bladder dysfunction across Tower Hamlets and within Barts Health. We recognize that urinary/faecal incontinence compromises skin integrity which contributes to the development of pressure ulcers and can also result in falls.

Visual Care Options - Mobile Optometrists
NHS eye exams are conducted at home for individuals unable to leave their homes independently. This could be due to a physical disability and also conditions like Alzheimer's or agoraphobia. Spectacles are issued when necessary with care provided on an ongoing basis. Feedback is provided to GP practices, social services, hospital teams and other referrers. Onward referrals are made to other health care teams or service providers if indicated. Visits are usually conducted at a person's own home, sheltered accommodation or care home. English and Bengali spoken.

Minor Eye Conditions Service (MECS)
We overview MECS (Minor Eye Conditions Service) in Tower Hamlets. This service is CCG/ICS commissioned and has been running since 2019. MECS was set up for accredited optometrists in the area to investigate and manage a range of common symptoms or conditions affecting the eyes. The service is free for those who are registered with a GP in Tower Hamlets. Notable benefits of this service include patients being seen quickly (appointment slots available within 48 hours), reducing chair time in both GP clinics and local hospital/A&E services. We work closely with Royal London Hospital and Moorfields Eye Hospital as well as GP practices and their representatives. Information and support for hospital, GP and optometry teams are provided when needed.

UCRS / falls prevention (Previous stall cancelled)
Admission avoidance team is changing name to urgent community response rehab service. We are integrated team of Rapid Response, Falls Pick Up and community rehab services in one. We are supporting patients independence in their own homes as well as able to provide urgent care input from nurses, therapists. We Have got community OPAT nurses who is able to complete IV antibiotics in patients home. This way we unsure any unnecessary admissions to hospital avoided, and patients are seen prior to referring to hospital care. We are also enable fast pick up for patients who are fallen, this way all unnecessary LAS would not be called. I believe our team should benefit from fair.

Tower Hamlets GP Care Group (GPCG) is a federation of 34 General Practices in Tower Hamlets caring for about 325,000 people. We provide a range of community and primary care services to the public, various 'at scale services' for the practices, and staff training through our CEPN and Open Doors services.

Health Spot
Health Spot is a specialist extended GP hub specifically for young people. It is run in partnership with Tower Hamlets GP Care Group, Docklands Outreach and Safe East. Health Spot is dedicated to supporting the health and well-being needs of young people aged 11-19 (up to 25 with additional needs or vulnerabilities). Young people helped co-design the service, which is delivered in Spotlight a safe and convenient space for young people. Health Spot offers friendly, confidential medical appointments with a doctor who is experienced in supporting young people with health needs.

The Hygiene Bank Tower Hamlets Everyone Deserves to Feel Clean
We distribute to local services who are in need of hygiene items for their clientele. Services include school, midwife, foodbanks, children and family centers, nurseries, youth groups etc. We receive donations from companies, grants, as well as having public drop off points at Boots stores in Tower Hamlets, which we collect and distribute for free to our community partners. There are currently three volunteers covering Tower Hamlets. Tower Hamlets Together
Tower Hamlets Together (THT) is a partnership of local health and social care organisations whose ambition and vision is to improve the health and wellbeing of its people living in the borough. The organisations that make up the partnership are NHS North East London (NHS NEL), Tower Hamlets Council for Voluntary Service (THCVS), East London Foundation Trust (ELFT), London Borough of Tower Hamlets (LBTH), Barts Health, Healthwatch Tower Hamlets, and GP Care Group (GPCG). There is a wealth of collaborative work being carried out across the THT partnership and some of its 23/24 priorities include: - Enhancing mental health & emotional wellbeing access & outcomes for children & young people. - Promoting care coordination & MDT working to improve outcomes with a focus on homelessness & frailty. - Reducing health inequalities in access, experience & outcomes. - Developing system wide health intelligence ‘data’ for localities & primary care networks & neighbourhoods. THTs’ staff engagement events are historic! Managed, developed and hosted by the THT People and OD Committee, events include its annual Summer Fair and Integrated Care Staff Awards. Care Technology in Tower Hamlets
Care Technology forms part of our Adult Social Care (ASC) offer in Tower Hamlets. ASC provides a statutory service to residents and carers, under the Care Act 2014. We have a duty to provide information and advice to help reduce, prevent or delay the need for ongoing care and support, as well as a duty to provide an assessment of needs, to safeguard adults and to support people to achieve their outcomes related to their eligible needs. ASC uses a strength-based approach to help improve the health and wellbeing of our residents.

Healthwatch Tower Hamlets
At Healthwatch Tower Hamlets, we are dedicated to being the champion for health and social care in Tower Hamlets. We value the feedback of residents, which we use to advocate for improvements in care by ensuring that NHS leaders and local authority decision-makers hear the voice of the local community via research studies, a patient experience program and Enter and View program. Additionally, we offer a signposting service that helps local people find and access the available services in the area as well as volunteering opportunities for residents.

Community Nutrition Support Dietitians (Food Fortification)
Come and test your knowledge of the energy content of everyday foods by playing our easy & fun game ‘higher or lower’ . Find out how this knowledge of higher energy foods ‘food first’ along with ‘food fortification’ can help optimise oral intake of those at risk of malnutrition or those with malnutrition. Find out how you can identify malnutrition using the simple ‘MUST’ tool (malnutrition universal screening tool) and have the opportunity to sign up for our September 2023 training session.

Greening the NHS
The NHS has a very ambitious target to reduce its carbon emissions. It needs all NHS organisations to work together to do this. The amazing thing about reducing the NHS carbon footprint is that it also links to other aims. In general, outcomes that are linked to healthier people are linked to a healthier planet e.g. clean air, healthier more plant based diets, more active people are less dependent on medicines, greener asthma care is actually better asthma care etc etc. We think that health and social care professionals need to be fully aware of the links and we are happy to raise awareness of this

Parkrun
Parkrun is a national organisation that provides weekly opportunities to walk, run or jog a 5km event. There are opportunities to volunteer at this event. Tower Hamlets has a parkrun in Mile End.

Doctor Digital
As part of our PCN IPO project we are trying to promote the NHS App, specifically for the purpose of encouraging patients to use the App to re-order their repeat prescriptions. We have a couple of patient engagement events planned and we are exploring other opportunities. There is a whole raft of work within the NHS to promote Digital Acceleration, encouraging the digitally savvy to access healthcare online. This then frees up time for Practice staff to help those 'digitally excluded' patients.

ELFT's Long COVID Service
ELFT's Long COVID Service is a therapy led service working with patients across Newham and Tower Hamlets. It is one of few service to work across two Boroughs and is for patients who have ongoing symptoms after 12 weeks of a COVID-19 infection. This event will provide a great forum to promote the service.

MIND Sakinah: Our Voice Project (Muslim Womens Project)
Sakinah: our voice project Is the peer lead project, serving Muslim women who are struggling with their mental health and well-being and who may not seek assistance due to the stigma or cultural reasons or either waiting to be seen by the NHS or have not acknowledged themselves that they require help or just need a little respite, it's a safe space for the women to come in and discuss their health and general wellbeing express how they're feeling and perhaps find ways and methods to help themselves and if not where we are able to signpost them to the right services it is almost like a waiting room for some to be seen by other health professions. Women have benefitted from coming to the sessions and are coming regularly, they have also invited new ladies to come and join with them. The sessions various from faith related engagement to arts and creativity to chitchat over tea and coffee.

MIND Advocacy Service
We are a mental health Charity Organisation, we are independent of the NHS Trust. We work in partnership with clients who reside in Tower Hamlets and who use mental health services. We support clients to feel empowered to express their wishes and concerns regarding their care and treatment. Our service is free, confidential and independent. We support those who are experiencing mental distress in accessing the appropriate support when they require this. Our work involves enabling clients to understand their rights under the Mental Health Act, explore options and give clients a voice. Our Independent Mental Health Advocates (IMHA), Community Advocates and Care Act Advocates will support as long as this support is required. MindTHNR also provides essential and valuable support and mental health activities, in reaching out to those who feel most vulnerable.

LBTH Reablement Service
The Reablement Team is an Occupational Therapist (OT) led, multi-disciplinary, integrated team. We have occupational therapists from ELFT and LBTH in our team, including an ELFT clinical lead. Our Reablement Officers (ROs) work with service users up to 6 weeks (or more, if identified), who have set goals that can be achieved within a period to ensure that they are back to their original baseline. Our ROs offer an integrated service; an OT across Tower Hamlets, whether working for health or social care, can request our ROs to work with their service users who would benefit from RO support to meet their goals. This has the benefit of opening our service to a broad range of OTs and not just the OTs working within the Reablement Service. We are involved in an integration project with the Admission Avoidance Discharge Service (AADS) and have been participating in Quality Improvement work to promote collaborative working and streamline services to prevent duplication and result in a better experience for the service user. As part of this project, the Reablement team provide the Social Work functions for the AADS team and we have a Social Worker funded by ELFT, though all three of our social workers work closely with Reablement and AADS service users. Moreover, AADS Rapid Response therapists can access our Out of Hours service (16:45-22:00 weekdays and 7:00-22:00 at weekends) to request urgent RO support alongside our in-hours service, to prevent hospital admissions for service users.

Our energy advice service offers impartial and confidential advice for people who are struggling to pay their gas, electricity or water bills to help you keep your home warm, stay on top of your bills and save money.

Tower Hamlets Early Detection Service
The Tower Hamlets Detection Service (THEDS) is a mental health prevention and promotion service in ELFT NHS Trust. We work with 16-25 year olds in Tower Hamlets who may be at risk of developing more complex mental health difficulties. Our small team is made up of a psychiatrist, clinical psychologist, a peer support worker, nurse practitioners and a community connector. We offer two years of wraparound support to young people with wellbeing plans and staying well plan through psychoeducation, regular medical reviews, case work, therapy and holistic support through peer support and community connector work. We use the 5 ways to wellbeing framework to offer support.

Tower Hamlets Recovery College
We are a non- hierarchical, co-productive, space of learning. We co-create courses that meets the needs and interests of people who live and work in Tower Hamlets. Who may have experienced emotional distress and the poor health conditions associated with. The courses are also for those who support people in the situations, whether through a professional or lay person capacity. All our courses are co-produced with people with first hand experience of conditions and situations which have impacted their health and those who have trained in a professional capacity to support them. We also offer one to one coaching via our independent learning plans. We have courses for young adults, people who speak Somalia and Bengali and for the wider community.
