Root Cause Analysis

Root Cause Analysis (RCA) is a structured method used to analyze adverse events. It can be used for any event, drug errors, falls, incidents of pressure ulcers and it can be used to help deal with complaints. When incidents happen, it is important that lessons are learned to prevent the same incident occurring elsewhere. RCA investigation is a well recognised way of doing this.
